Personal Care Services Policy and Procedure

Regulatory Reference: 12VAC5-381-360

Abid Health Care, LLC provides personal care services in compliance with 12VAC5-381-360.
These services are designed to assist clients with daily living activities while promoting
independence, dignity, safety, and quality of life.

Scope of Personal Care Services

Personal care services are non-medical services provided in the client’s place of residence and are delivered in accordance with an individualized plan of care.

Activities of Daily Living (ADLs)

Instrumental Activities of Daily Living (IADLs)

Medication Assistance

Personal care staff may assist with normally self-administered medications only when the client is capable of self-administration. Assistance is limited to reminders, opening containers, or placing medication in the client’s hand. Staff shall not administer medications or exercise clinical judgment.

Staff Qualifications and Supervision

Personal care services are provided by trained staff who meet agency hiring requirements, successfully complete background checks, and receive orientation and ongoing training. Services are provided under appropriate supervision.

Plan of Care Requirements

All personal care services must be authorized in a written plan of care that identifies the services to be provided, frequency, duration, and any special instructions or limitations.

Client Rights and Safety

Clients receiving personal care services are treated with respect and dignity. The agency ensures privacy, confidentiality, and protection from abuse, neglect, or exploitation.

Prohibited Services

Personal care staff may not perform skilled nursing tasks, administer medications, or provide services outside the authorized plan of care.

Documentation

The agency maintains documentation of services provided, staff supervision, training records, and any incidents or complaints in accordance with regulatory requirements.
